About The Summit: This year, we will focus on, “Demystifying Social Determinants of Health to Promote Healthier Aging in the 21st Century.” The two-day event will inspire a lively conversation and action-oriented agenda focused on addressing social determinants of health, as part of our broader campaign to tackle social isolation and improve the health and well-being of older Americans, especially those who live in rural areas.
Increasingly, stakeholders across the United States recognize that social determinants of health play an important role in understanding population health and improving health outcomes. Not coincidentally, we are starting to understand the serious public health threat posed by social isolation and loneliness, particularly for older Americans.
This year’s Summit bridge the topics to explore how social determinants of health impact older Americans’ social connectedness. We will explore how social and community support, food security, transportation, available health services, and other environmental factors influence older Americans’ quality of life and their social connections. The Summit will be an incubator for collaboration among diverse stakeholders to evaluate existing models of success and to consider new opportunities to address social isolation and improve the well-being of rural seniors.
