To refer a group of patients or clients to MSU Extension's Cooking Matters program, please download and fill out this form and return via email to cookingm@msu.edu.

MSU Extension encourages healthcare and other human services professionals to refer to our Cooking Matters program, when appropriate.
If you have a group of patients or clients (10-15 people; at least half of whom are SNAP-eligible) that would benefit from Cooking Matters, please fill out a referral form and return to cookingm@msu.edu.
