Every sample must be accompanied by a completed submission form. Please place the form in a separate resealable bag to prevent it from getting damp or soiled.

The total cost of testing can range from $20.00 to $100.00 for commercial production and from $20.00 to $50.00 for non-commercial production.

Please use the current forms, as all previous versions are now obsolete. Submission forms can be downloaded from the links provided below.

Clients from outside of Michigan: Please note that out-of-state sample submissions have a surcharge.

General Sample Submission Form

For plant health analyses and the identification of plants/weeds or arthropods (insects, spiders, ticks, etc.).

Nematode Analysis

Nematode Analysis Submission Form
- For all nematode analyses, including soil and root samples, foliage or bulb tissue, type testing, nematode trophic composition, and Verticillium dahliae testing in potatoes.

Herbicide Resistance Screening Submission Form

For the screening of herbicide resistance in weed seed samples.

MBC Blueberry Sample Submission Form

For Michigan blueberry growers only.
