Honor Clubs

For your Clare County 4-H Club to gain credit for the 2022-2023 4-H Year, you MUST complete this form.

This form can be filled out multiple times as needed throughout the year. 

Please keep track by indicating the month in which activities occurred.

To avoid duplication, please elect on representative of the club to report the activity here for credit. 

This is not just the responsibility of the Club Leaders to report. For example, if you are to be the only an individual from a club to attend a 4-H related event or activity, as that club's representative, please report so that your club gains the credit.
