4-H Club Forms & Information

Access Oceana County 4-H club year-end paperwork, forms, and helpful links below. Be sure to complete and return all required documents by the deadlines in your weekly 4-H emails. 
  • 4-H Club Fundraising Application.  The group treasurer or 4-H leader must complete and return this form to the 4-H staff in the county Extension office for approval at least 10 business days before any fundraising activities can be promoted or held.

  • 4-H Club Fundraising Follow up Paperwork. Complete and return this form to the Oceana County 4-H office within 10 business days of the fundraiser.

  • 4-H Club Civil Right Compliance Statement.  All Michigan 4-H clubs are required to sign the 4-H Club Civil Rights Compliance statement.

  • The Annual Financial Summary Report. -The Annual Financial Summary Report lists a 4-H group’s financial activities during the program year (September 1 to August 31) Please see to review to see which your club would need to use.  If you have questions contact the office at 231-873-2129.
